LICENSURE FORMS AND DOCUMENTATION

Please keep in mind that not all professional development submissions will meet LPDC criteria for approval. For further clarification from the Licensing Administrator, email [email protected]

 

Getting professional development opportunities pre-approved is ideal. Approval after the fact is not a given. 
Submission for pre-approval CAN NOT be made if more than six months have passed since the completion of the class/workshop.

 

Individual Professional Development Plan (IPDP):

Required for each new five-year license.

Plan entails three professional development goals taken from the Ohio Standards for the Teaching.

Begin goals with ‘I’ statements; reference the guide below.

 

Pre-Approval Form:

Required for each professional development course/activity.

Only 15 points of professional development can be earned per calendar year, from non-accredited teaching university/colleges. Max of 75 points allowed per license cycle: 15x5 = 75

               Example: Eureka Math, Writing Revolution, Make Math Moments, LinkedIn.

Include supporting documentation.

Course/Activity date should be complete [MM/DD/YYYY]

Professional Development opportunities MUST align with your area of licensure and IPDP goals.
